August 15 UPDATE** Player registration is now closed for the 2021-22 season. Late registration will only be offered in the age groups that are not full and will be a case by case basis. Please email [email protected] for more information.
**IMPORTANT NOTE** Please note that due to the current situation as a result of Covid-19, Seaforth Minor Hockey is not looking for payment at this time and that there will be no discounts for early registration until further notice. At this time we are waiting for guidelines from OMHA on what the 2021-22 season will look like. Player registration deadline has been set for August 15, 2021

BIRTH CERTIFICATES
If your child has NOT played for Seaforth Minor Hockey before, please provide a copy of the players Birth Certificate by attaching it to the Registration Form or send via email to [email protected]

- If registering 3 or more children with Seaforth Minor Hockey, the 3rd and any consecutive children within the same family will be registered at half price for teams.
- First time registrants for full ice hockey ages U10 to U18 receive 50% off the registration cost provided they have never been registered with any Hockey Canada program in the past.
ONLINE REGISTRATION:
Below are some helpful tips on completing your online registration form:
- When uploading to the Registration form, please note the only valid file extensions allowed are (.jpeg, .jpg, .png or .gif)
- If you experience any issues uploading your child’s Birth Certificate to the online registration form, please send a copy via email to [email protected]
- Parents/Guardian Section:
- The parent/guardian completing the form inputs their personal information
- The other parents address information (if different than the parent completing the form) will be input into the ‘Comment’ section.
- Ensure both parents first and last names are added to this section
- Example, John Smith and Mary Smith
- First Name: John / Mary
- Last Name: Smith
- Example, John Smith and Mary Hall
- First Name: John / Mary
- Last Name: Smith / Hall
- Phone Number / Email Addresses:
- Any subsequent phone numbers are to be added to the ‘Comment’ section. Please note which parent/guardian the phone number belongs too.
- Email Address: There are fields for two email addresses.
- Any subsequent email addresses required are to be added to the ‘Comment’ section. Please note which parent/guardian the email address belongs too
- For information as stated above
- Advise whether this player is a First Time Registrant (Novice – Peewee) and/or if they are a third or subsequent child registered within the same family

RESPECT IN SPORTS FOR PARENTS
At least one Parent/Guardian of every player MUST complete the online ‘Respect in sport – Parent Program (RIS)’ prior to the player’s participation. If you have completed this course and need to add a new player to your account, please follow the online instructions to “Add a Child”.
Every child must have this qualification on their file before they can step on the ice.
