                         Seaforth Minor Hockey Registration Refund Policy:

If you have opted to no longer play, are moving away, become unable to play due to injury or for other reasons, a refund request must be submitted and approved.

All refund requests require a submitted Request for Refund Form (completed online) to be approved. This form is located on the SMHA website under the 'Registration' section (or click on the green link for easy access).  All Refund requests are submitted to the Registrar. 

***All Refunds will be disbursed via cheque, mailed to the Contact person noted on the Refund form***

 

 BEFORE SEPTEMBER 30TH  Full Refund
 BETWEEN OCTOBER 1ST AND OCTOBER 31ST  50% Refund
 NOVEMBER 1ST OR LATER  Refund only on approval of Executive


Any special exceptions to the above table must be approved by the Executive Committee, as well:
     • For injury related refunds, the Executive Committee may request a Doctor's Certificate confirming the
        injury and the player's inability to return for the balance of the season.
     • For residential changes outside of SMHA's boundaries, the Executive Committee may request a 'Proof
        of Change of Residence' (sale of residence or Rental Agreement).
